Post-menopausal bleeding can manifest differently compared to cyclical bleeding experienced during a woman’s younger years. It may present as bright red bleeding, pink spotting, or even grey-brown spotting. Often, this bleeding is unpredictable and may occur without any accompanying pain, which can be surprising when one discovers blood in their underwear or toilet bowl. If you experience such bleeding, it’s crucial to consult your doctor promptly.

During your appointment, your doctor will ask specific questions, including when you went through menopause and when the bleeding began, as well as any additional symptoms you may have noticed. To prepare, consider these questions in advance. Additionally, some medications can increase the likelihood of post-menopausal bleeding, so if you are seeing a healthcare provider who is not your regular doctor, it can be helpful to make a list of your current medications to bring along.

Your healthcare provider may suggest a pelvic ultrasound to determine the reason for the bleeding. This ultrasound provides a thorough assessment of the uterus and ovaries, helping to define the underlying cause of the bleeding. It evaluates the uterus, ovaries, adnexa, and vagina, all of which are important in diagnosing the issue. Using a trans-vaginal transducer often yields higher-resolution images of these structures, which tend to be smaller compared to those in women who experience cyclical bleeding.

One of the key aspects of the pelvic ultrasound is assessing the lining of the uterus, known as the endometrium. A thin endometrium can be reassuring for both you and your doctor, potentially helping you avoid surgical procedures or an endometrial biopsy, both of which come with additional costs, discomfort, and risks. Obtaining a clear image of the endometrium is crucial in making an informed assessment.
